Program Requirements
• Photocopy of passport
• Letter of good standing (students) or proof of completion (graduates) from your medical school
• Personal health insurance
• Medical malpractice (liability) insurance (included in placement fee starting January 1, 2012
• Immunization records--Immunity to Hep. B, MMR and Tetanus; Negative PPD results in the past 12 months. If your skin test is positive, we require a report of a negative chest X-ray reading in the past 12 months. PPD's must be completed in the past 12 months.
• A police report from applicant’s hometown police department, OR a complete criminal background check, available online from a number of sources.
• Signed waiver form (sent to applicant upon acceptance)
• Verification of ECFMB (instructions sent to applicant upon acceptance)
• Participation in Chicago Clerkships' applicant orientation and HIPAA training (conducted by telephone).
Timeline
Please allow a minimum of one month between your application submission date and your proposed placement start date.
Within 2 weeks of application submission
• Applicant will be advised of acceptance/denial.
Within 2 weeks of submission of required documents (above)
• Applicant will be notified of clinical placement
• Applicant will receive information about homestay placement options if requested.
• Applicant will receive letter in support of visa application (upon request).
One week before rotation placement begins, applicant must :
• Participate in orientation session (via telephone)
First day of rotation placement begins, applicant must :
• Submit payment via credit card, cash, or money order from U.S. bank.
